Severe abdominal pain with Flare up

6 years into IBS, fodmap doesn’t work I react to the okay and bad foods and now with flare ups (30minutes-1hr after eating) I’ll have extreme full abdomen and back pain, sharp left side cramping and gastric. But for 4-5 hours after I’ll have extreme abdominal pain that enables me from walking and non of my pills work. Anyone experience this .
FYI- had every test and surgery so I’ve ruled out IBD
