Hey there, I'm back - and I'm a very special case now :-P

I've been gone for a while (I needed a break from everything related to this topic).

I wanted to write down what happened to me because I still haven't given up hope that maybe someday someone will be like "hey, I recognize this!" (also seeing a lot of different doctors now but none could help me so far).

For the longest time I thought I had SIBO (and I maybe still do). My main problem is that while I maintained a pretty restrictive but manageable FODMAP-diet for years (using some probiotics), I have been losing more and more safe food options over the last 3 years now.
I'm currently down to white rice, olive oil, carrots and poultry. Thankfully a good doctor also made me take vitamins and I'm now taking D3+K2 (sublingual), E+A (sublingual), Folate, B12 (sublingual) and Vitamin C (as Calciumascorbate which is much better tolerable than the acid version).

After an antibiotic I had to take for a severe case of sinusitis like 3 months ago, I'm now constantly having dirrhea (every second day more or less). So my doctor checked my stool because I also developed burning in my intestines and I have a Calprotectin (inflammation marker) of 800 which speaks for a IBD (Crohns or Ulcerosa).
I think: Dandy! Finally something that can be diagnosed so I'm basically up for a coloconoscopy 2 weeks later (thank god my doctor at least tries to help me) and my result does indeed show inflammation (large intestines looks like its full of mouth aphta and my small intestines and my valve have more round-shaped inflammations) ..... but it's neither Crohns nor Ulcerosa according to the lab and my doctor - who has not found a single picture in his book matching my large intestines. Best bet is something bacterial or a virus? (who knows maybe Corona?! Wouldn't that be peachy 😁)

So I run around with a severe malnutrion, have constant gurgling in my stomach if I lie down and can't eat foods anymore. This is currently a very annoying/tricky situation and I feel like I'm out of options.
Currently I'm (self-pescribed) Sacch. Boulardii (cerevesia - the probiotic yeast) because I read everywhere that this probiotic at least fights inflammation and protects the intestinal lining.

Anyone here any ideas reading this?

  1. Hi , I'm happy to see an update from you, although it's sad to hear that you're still dealing with all these symptoms and have inflammation that the doctors can't explain.
    Have you tried asking the IBD community (https://inflammatoryboweldisease.net) if they have ever heard of inflammation like the one you have?
    If it's bacterial or viral, shouldn't it be possible to test for that, or do they think it's some sort of long Covid or something similar?

    1. Stool test came back and I have indeed a very extreme Klebsiella and Enterobacter population in my stool which is not healthy. How I should react to that I have no idea. My practioner said that throwing more antibiotics on those is a very bad idea and I should consider probiotics instead to fight them. Rn I'm taking the Boulardii (no real improvement after 9 days) and I'm eyeing these:

      3 Bifido/3 Lacto species that have all (I have checked) been checked with "Leaky Gut" and "IBD". This probiotic is for children and is activated in warm water.

    2. Oh no! 🙁 But at least it gives you some idea of what might be going on...
      Did the doctor explain why antibiotics would be a bad idea? Is it because it's more of a dysbiosis rather than an infection? I'm not familiar with these bacteria, so I'm just wondering.
      The children's probiotics sound very promising, it might be worth a try.
      I've also used Lacticaseibacillus rhamnosus recently after getting sick and it worked amazing, it's used after antibiotics as well.

  2. I'm sorry you are feeling so bad, and there appears to be no answer, as even your results can't be understood properly by doctors!

    Saccharomyces boulardii is a brilliant choice of probiotic though, because it's supposed to help with diarrhoea after antibiotics. I have heard it has had good results with many people. The interesting thing is that it's a yeast-based probiotic rather than bacteria-based. And it's single strain. whereas a multi-strain with high levels of bacteria can ne too much for some people.
    It's great that you have some good supplements to take daily owing to severe dietary restriction.

    I have also heard that it's possible to re-train the microbiome to accept a wider range of foods by having the tiniest amount of a food, awaiting the results in 2 days or so, trying it again and very gradually building up the amount over a period of time.
    I usually fail at that though, as I will either get ravenously hungry and ignore the measurements (I always feel fine at dinner time. It's often the next morning I pay the price.) Or I will buy something like an orange for example, and intend to eat half of a segment three times a week, building up to a whole segment, and so on....and the dratted orange will go off in the fridge or I will forget it's there.

    1. I'm now 9 days in and so far it has only slightly gotten better (poop at least not water-consistency). I wonder if I have to wait longer ... I can feel my intestines working - maybe that's a good thing?! I dunno.

  3. It's probably pretty normal to feel your intestines working, Only those of us with IBS get a bit over sensitised to those feelings. But before I got IBS, when I was well, I do remember sensations like that and they didn't mean anything bad and I didn't even think much about them.

    It's a good sign your poops are firming up slightly...even a bit.
    I don't know if you can handle this food but I always find plain white basmati rice (with just a bit of salt) will almost always definitely firm up bm's for me, especially if eaten with some protein with 0 fibre, such as plain cooked fish or boiled eggs in my case, but I am sure a white meat like chicken would work too if you eat meat.

    Once I get stabilised like that, and come out of the flare up, often I can start to introduce other foods such as steamed vegetables in very small amounts and gradually.

    But I find I mustn't overdo it with "binding" foods such as I mentioned, or I will swing over to constipation. I just eat those things for a couple of days or until I am stabilised.

    Antibiotics are so harsh on the gut. I remember my tummy bouncing back well from them in the past with no fuss at all, but IBS changes things doesn't it? We can become so super sensitive to an onslaught like antibiotics and it takes ages to straighten out.

      Yeah, I feel really bad right now. I'm currently debating the ongoing use of Boulardii. Gurgling was very well tonight, I just woke up once (something like a nightmare, calmed again and went back to bed after meditation). Today I had one of the most shameful experiences ever tho ... after an almost "normal" (but very lightly coloured stool) I was on my way to the library when I had a very bad onslaught of dirrhea ... I had to take it in the bushes .... I feel so low right now.

      Eggs I would love to eat but they give me gas like crazy (almost everything does nowadays).

  4. Oh dear, I am so sorry you had that humiliating experience today. It can make us very scared to do anything or go anywhere! Plus it's not easy to clean up when outside.
    I don't think you are alone in those kinds of Experiences. I read an article here a while ago by one of the Team, which described a similar happening!
    It can help if we have an emergency kit in our bag with wipes, maybe a deodorant spray, a non-leak plastic bag for any possible dirty underwear etc, and a clean change.
    But even so, it's still horrible.

    So I guess S. Boulardii isn't helping much? Or maybe it helps a tiny bit? That's a disappointment as I have heard good things about it for antibiotic-related diarrhoea.
    I don't know as I have not taken it myself, but I wonder if taking it for a while longer might start to show results?

    I hear you about eggs. They are good for me and help in my arsenal of "binding foods", but they don't suit everyone.

    I am guessing you've already tried experimenting with the dosage of Imodium? Not so much that you get blocked up, but maybe find a "sweet spot" dose? Or maybe that doesn't work.

    1. Hey there - no, I canceled my Boulardii experiment today after 10 days. Maybe I'll try again someday but so far it has made the situation more or less worse (which is kinda ... strange). It really feels like what my intestines need is to calm down a little (inflammation) and then a "good" probiotic. I found one (Probaflor Junior) with 3 Lacto/3 Bifido which is labeled for "sensitive people". If that's true I don't know. I'll not try that for another 1-2 weeks anyways (can't take everything all at once).
