IBS-D ,A sharp worsening of symptoms

26 year old diagnosed with IBS-D since age 18.

Symptoms: on a daily basis 8-10 visits to the bathroom, soft feces-texture diarrhea, severe abdominal pain and distortions in the upper part of the intestine with radiation to the entire stomach area.
The stomach is felt during all parts of the day, on average once a month there is a severe flare-up of the intestine that can last from one to two continuous weeks of pain that does not stop, at the level of complete shutdown, an accelerated pulse, very severe nausea, diarrhea, inability to eat a full meal, reflux and a desire to vomit during and before eating, Hair and weight loss, in attacks a sharp weight loss up to a 1 kg in a week - week and a half.
In the last two years, there has been a sharp worsening in the frequency and duration of the attacks, to the sharp pain attacks, cases of perineal abscess began to be added regularly!
I underwent two abscess drainages and then it continued for a period of several months at the same time as sharp pains in the stomach and I continued treatment independently with ointments at home.
Today, the abscess has not returned to me for six months with the attacks with the same frequency and the pain is really getting stronger.

Treatments: In the past, a gastric ulcer was diagnosed and treated, Helicobacter bacterium three times, Sibu bacterium treated with Lorimex.
A lot of painkillers were given, such as Zaldiar, antidepressants, altrolate, and the like, of course, which are antacids on a regular basis, but do not improve the condition.
None of the pills helped me and managed to soothe the source of the pain
Uses cannabis pain attacks which helps to calm down the source of the pain for a certain time.

Tests: Over the years, comprehensive screening tests were performed for the abdomen with a real fear of intestinal disease, such as a colonoscopy and gastroscopy three times and a normal biopsy, an endoscopic camera test, a comprehensive abdominal CT including the liver, ruled out chlorptactin in the feces, an abdominal ultrasound, and more, all of which without any unequivocal diagnosis of intestinal disease or diagnosis to the source of the pain apart from a definition of "irritable bowel symptoms".

In which direction would you recommend going to check the source of the pain? How is it logical that no pill has any effect and the pain lasts 24 hours a day, Unable to lead a full life routine.
I would appreciate any help or direction for further testing that could improve the situation for me

  1. Hi I’m really sorry to hear how much you are suffering with your symptoms. With my symptoms causing me frustration to no end, I have been researching all sorts of remedies and potential treatments. I read that Anti-depressants was useful in reducing (to the extent of eliminating) the symptoms for some people. But it sounds like you’ve tried that to no effect. However, more recently I have been reading about something called ‘psychobiotics’. These are prebiotics that re-balances the gut-brain axis and the role of the microbiome as an important factor in modulating gut-brain signaling.     1. Hey there. You have some really severe symptoms 🙁 Sad to hear that you feel so unwell.


      What I'd recommend to you on the spot (because it at least can't make things worse):
      - meal-spacing (3 meals a day, 4-5 hours apart) (helps with digestion)
      - gentle self-induced stomach massages (helps with digestion)
      - slow walks for at least 5 minutes shortly after every meal (helps with digestion)
      - meditation (helps with stress and anxiety)


      I also would be curious if anti-depressants in very low doses would help you (as the person above me mentioned).

      1. Thank you for the answer, everything you mentioned has been done, I should mention that I also did a Food Map diet with a clinical nutritionist who specializes in IBS-D without a benefit

There is nothing that goes into my mouth in this condition, I have been on vegetable soup and white rice for a week with pain in the back of 10/10 distortions in the entire intestinal cavity it comes in fits and doesn't stop, regarding nutrition I have been very careful for years about what goes into my mouth and check and read everything terrible food slowly. But in the situation I am in right now, in a pain attack, there is no ability to eat, a lot of reflux, weakness in the body and a very sharp drop in body weight
